Service Description: To apply for our services you have to receive an ‘assessment’, which helps us to decide what you need. We have a legal duty, under the NHS and Community Care Act 1990 to assess adults who we think may need Adult & Community services.

It gives you the chance to tell us what you are good at, and what help you need to makes things easier in your day-to-day life.
