CAF Guidance

This guidance document is for practitioners who may want to initiate the CAF process for a child or young person (0-19). The document outlines the processes of preventive services and specifically the introduction of the CAF and lead professional role.
CAF completers will need to make themselves familiar with this guidance before undertaking a CAF

CAF Form

The CAF form is designed to help practitioners record and, where appropriate, share with others, their assessments, plans and recommendations for support. The form is available as an editable word document so it can be downloaded, and completed, but the framework and content of the form must not be altered in any way. The local CAF Steering Group have agreed slight additions to the national CAF form to improve the consent section. When completed please email the CAF form to the secure CAF mailbox: caf@iow.gov.uk.. Please note signed consent (page 9) will still need to be sent in separately. If sending in the CAF via post the envelope should be marked CAF and please adhere to the protective marking policy.
