Referrals

At the moment the Isle of Wight Support People Team does not accept self-referrals to any of the Island’s Supporting People services. Instead, they would need to be put forward by someone such as a Care Manager or Health Worker or representative from one of the key agents listed below.

All referrals for Supporting People services are made using the NCR (No Carbon Required) referral and assessment form issued by the Supporting People Team. Each form has an identifying referral and assessment number which is tracked by the Supporting People team.

To carry out a referral and assessment you will need to be either a:

  • Care Manager
  • Duty Social Worker
  • Mental Health Practitioner
  • Probation Officer
  • Housing Officer
  • Supporting People Team
  • Other Approved Referral Agency
