  • First Response – the first point of contact for new referrals to adult social care.  For more information, please click here.
  • Self Directed Support – help adults if they are likely to need on-going support in order to live life as independently as possible. The team will do this by helping people through the Personal Budget application process.  For more information, please click here.
  • Long Term Conditions including Memory Service – help adults with a long term condition or a complex unstable need.  For more information, please click here.
  • Review & Quality Assurance - ensures all people receiving support via Adult Social Care are regularly reviewed.  For more information, please click here.
  • Hospital Social Work - help to facilitate timely and appropriate discharges for all referrals accepted.  For more information, please click here.
  • Mental Health - work with people aged between the ages of 18 to 65 who have a severe and enduring mental illness.  For more information, please click here.
