This transfer plan describes the process of how the SEN team will transfer your son or daughter over to an Educational, Health and Care Plan over the next few years.

The timetable shows you how many Statements of SEN will be transferred to an Education, Health & Care Plan over the next three years.  If your child is in one of these year groups, your school will be in contact with you to arrange a date for a transfer review meeting.

A transfer review meeting will be held instead of an annual review meeting and information will be gathered from you, your child, the school and other services that support them. This information will then be sent to the SEN Assessment and Review Team and the transfer process will be started. This process should be completed within 20 weeks if the transfer is from an SEN Statement to Education, Health and Care Plan. This will also be the case of a transfer from S139a or S140 to an Education, Health and Care Plan.

You will receive a draft of your child’s Education, Health and Care Plan before it is finalised so that you can see what has been written about your child, their special educational needs, the special educational provision, school placement and any health or social care provision that your child needs.
